It is most common complaint of Great Britain and digestive affects one third of the population at some point in their lives.

Twice as many women are affected by Irritable Bowel Syndrome (IBS) as men – more than one in five at a time.

But patients complain there is little effective treatment for the disease.

But now a new drug may be about to bring new hope for one in four patients.

The treatment – called Tegaserod until its mark is determined – is based on research linking the disease with chemical serotonin in the body naturally.

It is currently on trial and if approved by the Medicines Control, an agency that licenses all drugs in England, could become available by prescription in October.

IBS is a chronic disorder of the intestine that can lead to depression, nausea, embarrassment and constant tiredness.

Classic symptoms include severe abdominal pain, diarrhea or constipation, stomach bloating, headaches, back pain and excessive wind.

While some people suffer all these symptoms, patients are usually divided into three groups: those suffering from diarrhea, constipation, those who suffer and those who suffer.

The new treatment should be particularly useful for patients who suffer badly with constipation.

Although most assume the disease causes only minor health problems, hundreds of patients complain that IBS affects your ability to lead a normal life.

More than two fifths of sufferers say it affects their ability to work, live, travel and eat certain foods. The condition can even affect their sex lives.

Typical foods trigger IBS are associated with high-fiber foods and spicy, like curry and cous cous. But patients say nothing of dairy products to fruits and vegetables can trigger an attack.

Hundreds of patients have symptoms so severe they are hospitalized in the UK each year. Moreover, he was known by sufferers, driven by a horrible pain to commit suicide.

Despite the large number of patients, yet very little conclusive research has been conducted on the causes, diagnosis and treatment of IBS.

However recent studies on serotonin – one of the body’s natural chemicals thought to aid digestion – is proving to be a breakthrough for sufferers of IBS.

Dr. Alasdair Forbes, a gastroenterologist at St Mark’s Hospital, London, who is conducting the tests, says the new treatment will help regulate certain chemical processes in the intestine.

The new drug is said to stimulate serotonin receptors in the gut. This has the effect of relieving pain and stimulates the bowel to work more quickly.

Although the drug is not a miracle cure, Dr. Forbes says he was found to improve symptoms in 15 percent more than other conventional drugs.

“We believe that one in four patients with the syndrome will benefit from the drug,” he said. “Serotonin is an important advance in IBS research and is beginning to bear fruit.”

Dr. Forbes admits IBS sufferers are often dismissed by doctors, but says the future is looking brighter.

“There could be an effective treatment on the horizon, and finally, the suffering is being taken seriously by gastroentologists,” he said.

A spokesman for the Control of Medicines, said: “If the safety and efficacy of the product meets the relevant criteria required by the MCA, then it will be granted.”

The new drug has the endorsement of the digestive disorder Foundation.

“This is a very exciting development for people suffering from IBS,” says spokeswoman Rhonda Smith.

“The evidence so far suggests it could be an effective treatment in the future. However, the drug is suitable for people suffering from constipation dominant IBS. It is important to note that not all IBS sufferers will benefit from the drug.”

Traditionally, IBS has been conceptualized as a condition of visceral hypersensitivity and gastrointestinal motor disturbances. The gastrointestinal motor disturbances identified, including changes in intestinal transit, do not easily explain mixed or alternating IBS. Some have suggested that these abnormalities are secondary to psychological disturbances rather than being of primary relevance.

However, not all patients with IBS have significant psychological overlay and referral bias may partly account for the psychological associations. Hints as to why visceral hypersensitivity and gastrointestinal motor disturbances may arise are emerging. There is now convincing evidence that IBS can arise after bacterial gastroenteritis in up to one-quarter of cases.

How often subclinical cases of gastroenteritis account for those who develop IBS without a history of preceding gastroenteritis is unknown. There is also accumulating evidence that a subgroup of patients labeled as IBS have subtle inflammatory bowel disease, although the exact prevalence of these findings in IBS overall remains unclear.

Mast cells have also been observed to be increased in those with non-postinfectious IBS based on quantification methods. Using conventional histological criteria, these biopsies are normal. Hence, we have probably been overlooking subtle inflammatory bowel disease in IBS in the past. Thus, there are subtle inflammatory changes in colonic biopsies in IBS.

There is also evidence that there is a change in peripheral cytokine profiles in IBS reminiscent of those seen in inflammatory bowel disease. For example, an abnormal interleukin (IL)-10/IL-12 ratio has been observed in IBS, suggesting a pro-inflammatory state. Based on evidence such as this, application of the label functional to IBS now seems rather inappropriate.

Whether inflammation can alter mucosal control of motility is an area of active research interest. Serotonin, present largely in the enterochromaffin cells in the gut, is a major regulator of the peristaltic reflex and sensory relays in the gut. Two lines of evidence support the view that serotonin regulation is abnormal in IBS. The release of serotonin in plasma appears to be reduced in those with constipation-predominant IBS and increased in diarrhoea.

In a seminal paper, rectal biopsy specimens were assessed in patients with IBS, ulcerative colitis and controls. A defect in serotonin signalling was noted in both IBS and ulcerative colitis, with a reduction in normal mucosal serotonin and serotonin transporter immunoreactivity in both diseases. This implies that there may be a real molecular defect in IBS, which conceivably is acquired possibly after infection.

IBS often disrupts daily living activities. 19% of respondents in a survey of married or cohabiting people with IBS stated they had difficulties in their personal relationships, and 45% stated that it interfered with their sex life. The need for effective IBS treatment is therefore of a high priority. Symptoms and their causes are what any treatment for IBS must address immediately.

Many diseases are similar with the same symptoms as IBS. Some of these illnesses are serious and require aggressive evaluation and treatment. A differential diagnosis for patients who present with abdominal pain and altered bowel habits is summarized. To date, no gold standard or marker for IBS exists. A cost-effective diagnostic approach that uses the fewest tests and invasive studies is most desirable. As in all illnesses, the most valuable initial tools are a detailed history and physical examination. If alarm symptoms that suggest an underlying organic disease are uncovered, further testing usually is considered.
Scoring methods, subgroup classifications, laboratory studies, endoscopy, and psychiatric assessment are available to help guide the diagnosis in patients who present with abdominal pain. Several scoring systems for diagnosing IBS have been proposed. No system is 100% sensitive or specific for IBS. These scoring systems, which still are being validated, are useful for research and can help guide the diagnostic evaluation. The Manning criteria have the greatest number of validation studies. An expert panel recently took the most validated elements of the Manning criteria and broadened them to create the Rome II criteria.
Patient subgroups can be clinically useful. These subgroups will be discussed later in this article, where a diagnostic approach for patients with abdominal pain and altered bowel habits is outlined. Blood studies can point to organic causes for pain and altered bowel habits. Most doctors suggest that all symptomatic patients have a complete blood cell count. Determination of the erythrocyte sedimentation rate, thyroid-stimulating hormone level, and electrolyte levels is useful in patients with constipation-predominant and diarrhea-predominant symptoms. Fecal occult blood testing and the testing of stool for ova and parasites are useful in patients with diarrhea.
Lactose-malabsorption studies have limited value except in patients with diarrhea-predominant symptoms. Patients with IBS have a structurally normal colon. Flexible sigmoidoscopy adequately screens most patients who present with abdominal pain and altered bowel habits. Complete colonoscopy should be considered for use in patients older than 50 years and those with alarm symptoms. Abdominal ultrasonography is not needed in patients with IBS because it can lead to overaggressive diagnosis and treatment of minor findings. No psychometric screening tools have high enough specificity or sensitivity to warrant their use for diagnostic purposes.
In some cases, a psychosocial evaluation is recommended. No single diet can work as an IBS treatment, as cases vary from person to person. The intestines are very sensitive to stress and can exacerbate IBS. Adequate sleep with regular meals and exercise is important when going through IBS treatment. Restriction of alcohol and tobacco is often necessary to treat irritable bowel syndrome properly. If you are one of the people who are diagnosed with irritable bowel syndrome, you undoubtedly have questions as to how to live with this situation. There is increasing evidence that supports the view that IBS is a disorder of brain-gut function. The brains ability to control motor activities of the intestines/gut is impaired. There is an imbalance between the stomach, intestines, brain functionality, and autonomic nervous systems. Actually, three main factors are significant in the pathogenesis of IBS.

Colonic motor activity is abnormally high in patients with colonic pain, particularly after meals or emotional stress. Many with IBS exhibit symptoms of colonic imbalance at times of stress which is commonly associated with a nervous disorder and anxiety. IBS has been linked to food allergies. Therefore, a food sensitivity test may be helpful. Foods which are reported to provoke irritable bowel syndrome symptoms include wheat, dairy, caffeinated anything, citrus fruits, and alcohol. It may surprise you that by following an IBS diet, you can help keep your IBS under control.

One of the easiest changes that can be made to your diet is to avoid eating foods that are known to cause gas, for example, broccoli and beans. Beware however, that foods are not the only thing that can cause gas, apple juice and grape juice can have the same effect on your body. Another IBS diet secret is to stay away from caffeine, sorbitol and alcohol, all of which can aggravate the bowel and cause symptoms to flare up. It’s best to pass up high-fat foods such as pastries and other desserts for the same reason.

Irritable bowel syndrome is a clinical condition characterized by a combination of abdominal pain or cramping; change in bowel pattern change of consistency of bowel movements; increased secretion of colonic mucus; dyspeptic symptoms which include abdominal discomfort, bloating, gas, a feeling of unusual fullness with very little intake of food, nausea, loss of appetite, heartburn or belching; varying degrees of anxiety or depression. Several names for these common disorders have appeared over the years.

These include nervous indigestion, functional dyspepsia, irritable colon, spastic colon, colitis, functional bowel disease, and mucous colitis. Colitis is an inflammatory condition of the colon. It often strikes young and early-middle-aged adults. Symptoms include cramps, diarrhea, and an almost continual need to use the toilet. There are several types of Colitis, ranging from mild to serious. Nineteen percent of respondents in a survey of people with IBS stated they had difficulties in their personal relationships, and forty five percent stated that IBS interfered with their sex life.

In conclusion, IBS often disrupts daily living activities. The first property we apply to treat IBS is the levels of influence a select few medicinal plants exhibit against IBS.
